If you tell someone at work they are fucking up and costing the company money at what point should you throw them under the bus? Or just wait and let it hit them eventually?

What he said. Depends on your positions relatively, the C2 hierarchy, and what your lane is. In most jobs leaving your lane, regardless of intent, is worse than being a fuck-up.
