You may have already ruled this out but are you using EVGA Precision X for any overclock / custom fan profile? There's an outstanding bug that causes the video driver to crash when Precision X ramps up the fan on a custom profile. I stopped using it 3-4 months ago and haven't crashed since.

Yeah, EVGA Precision has an unresolved bug that causes random driver crashes when custom fan profiles are used. I'm not sure if it's more prevalent on SLI or certain overclocks but it was killing me in Witcher 3 with 2x 780 FTWs. So if you must overclock / custom cool, use MSI Afterburner.
