He met him while he had a beard. You couldn't see his scars. He also talked to him for a few minutes and had no reason to believe the guy mowing the lawn had any connection to this at that point.

My main problem with the theory of Marty making up the doll scene in his head is that it then creates complete doubt in all the flashback scenes. I believe the flashbacks are exactly how the events happened from an unbiased narration. We're not seeing things how Marty or Rust perceived, but...
