It is from the behind the scenes video cdpr released for the witcher 4 reveal trailer. Maybe it is just different lighting as some say, but her face does look to have a thinner bone structure.

It really could be the same model. Softer light from a different angle, head tilted differently, mouth closed.
Would actually be pretty funny if it's just those little difference that make the difference between looking like anaphylactic fan-art vs W3 Ciri aged up.

Think the combat will be better than in 3? I recently tried, for the third time since the game’s release, to play it and I absolutely cannot stand the combat. Hope the fourth one takes a different approach and doesn’t suck in other ways.
