If the first 5 pk are missed by both team, 5 other shooters are then put on a list and I beleive it's sudden death. If they all miss, the keepers shoot. If they miss, you start over in the same order (still sudden death). No coin flip ever involved. A couple years ago there was an Africa's Cup match (semi final or final) with Ivory Coast where they needed like 15 shooters. Not because they were missing, but because they were all scoring!
