Assuming a normal CableCard style decoder setup - UVerse, Dish, and a few others aren't in the game - only FiOS/Comcast/DirectTV can cablecard to my knowledge.

I'd think Comcast will be ok with it - they WAAAAAAY overcharge for cablecards which you need to translate the cable signal for other boxes. $3/card for the same card on FiOS now that I was paying $10/mo for on Comcast. (Plus MS/Comcast were in bed with each other until recently)
