Geez, what ilvl are you at that you're getting failures already? I'm on my phone, but my guess would be it wouldn't be until about 440 that they start failing, but the percentage is super low.

I got super lucky with my stuff so far it feels like. Won't matter in a week, but it's a nice start. Got a stone with 2 engravings that I needed, and got it 4-5 (had a 2nd to try too, but haven't yet). And the 4 one (super charge) I have an accessory with +1, so I'm sitting at 1-1-1 engravings.

From what everyone has said, the population doesn't really matter. Zones will get instanced after about 30 players, with potentially hundreds of instances of the same zone on a single server.

Depends on where you go, but I've seen literally every pack listed as the best value and worst value. Except platinum which people all say is worth it if you're willing to plunk down the cash.
