Dying before the first boss is considered an ending, that's why it makes you restart. You can also remove stuff from the skill menu that kills you which is also considered an ending. Lot of quirky stuff in the game.

Yeah that rubber stopper/foot thing pushes something near the eject button that causes it.

Mine used to do that and so did my roommates. Pull out the rubber stopper on the bottom/middle and it will fix it.

Ordered with Prime and it got the discount when I checked out. This is one game I am really on the fence about canceling since not sure if it will live up to expectations... prime discount is the only reason i per-ordered.
