Kinda like old necro in dota. You had an aura that caused hp loss, but could do an aoe heal/damage attack also. If the aura was good enough, it could cancel out aura heal stacks. Only issue with aura damage is it would just encourage heal stacks to counter it.

I liked it, because it gave a reason to rerun content. If I get a regular trinket of poop, but it has the chance to drop as trinket of titanpoop, then I might be more motivated to continue to try my luck.
