On my necro I went to oasis for orcs/crocs when I got bored of sk then rathe mountains for undead then I switched to giants when I out leveled those. Giants weren't overly camped when I was grinding on p99, don't know what its like now. Kunark is the best bet for faster leveling though...

Did you try EQ Atlas? The old maps are out there and its pretty easy with all of the paths and other landmarks. If you can't cast invis or can't afford pots then run through loio, preferably after begging a sow in FoB. I can still remember how much I hated the run from Cab to Dreadlands...
