Only gamble weapon, amulet, or ring if those slots have pieces that are absolutely critical to your build and you have nothing else that needs gambling. The variety(both in legendary gear and possible affixes) and shard cost makes gambling those very inefficient.
Basically anything that abuses 4pc sunwuko. There are a few variations within that, just remember not to use any generators AND spenders with attack animations. One or the other.
Lightning with tons of procs or cold with rimeheart/fist of azturasq come close but not as good as above.

Quite a hamfisted "fix". I'm not really complaining, got my first ever rorg last night. But I don't think this going to make people want to run bounties, not in a balanced way. They'll binge them to get their rorg, then only run them when they need rift keys.
I would recommend using FoF in that build, even though it's the wrong element. I did some DPS tests, you can see my gear in my sig, and FoF killed Ghom faster by ~7s each time. Obviously, more lightning% will favor FoT, but I don't think you can get enough to overcome FoF's proc coefficient...

The patch changed very little for crusaders. I've never been a fan of the fire rune for blessed hammer, it just exacerbates the windup time already inherent to the skill. I prefer the holy rune. But if you like the play style, yeah, that will work for a while. It won't be an endgame build...
That's a really time inefficient way to farm though. I mean, if you've got something to do for 5-10 minutes while you let the timer count down, go for it.
I know of that thread, posted in it a few times. I don't remember it ever describing proc coefficients the way you have. The first post certainly doesn't. Can you be more specific as to where they specify that?
On the contrary, the top monk build doesn't use "spenders", as in TR, WoL, and LTK, because they interrupt other attacks. Everything is geared towards spirit generation and pumping out clones.
I'm slightly confused why I haven't seen more high level monks using that gem. Especially with FoF, it's essentially 4% max life per target, which is a ridiculous amount of healing. Surely that's worth more than whatever your 4th ring primary is.
Not a good way to look at it. You get damage stats if you can. If you have leftover primary affixes, like on chest, shoulder(maybe, spec dependent), legs, boots, belt, and bracers, you get VIT. Got more leftover? Get RA. Then life%, then armor.
Depends on class and spec and gear. Which is really how it should be.
I know for monks, both setups are viable. Monkey King set pushes daibos ahead at the high end.
Torment setting is irrelevant to grift difficulty.
As for when to do them, do them if you have gems that need upgraded levels. Best difficulty? Lowest difficulty that allows you a good chance to upgrade the gems. Even if you're gear is pretty good, you'll upgrade your gems quickly enough...
I think it's a federal law, or most states, that they have to remove PMI after a certain percent too. It's like 1 or 2 percentage points past 20%. IIRC, your credit score(which your history affects) doesn't affect PMI but your lending rate.
