If you remove the trash and key requirements other MMOs put in to pad kill times FFXIV has the hardest raid content.

Not sure if it takes the crown as a_skeleton_02 suggests, but I can say it's arguably the most dramatic. When Ultima Weapon (Ultimate) came out, everyone got suckered into realizing the Woken mechanic was required and not a buff on the boss to be avoided. As Mister Happy - I think it was him - put it: "THE PAST TWELVE HOURS WERE A LIE." Everyone had to start from scratch, basically, and I about wet myself hearing about it because I'm a sadist for watching this stuff.
Of currently online games. IIRC WildStar was harder which was a big factor of how it died.

I was having video display issues with my new TV plugged in as a monitor until I disabled all App/Smart/autorun bullshit and zero troubles. Literally malware. Also the 14 client really hates changes to video/audio. But the smart shit is still malware.

That's a sexy enrage. We need more of those. It was a long time before I found anyone who posted (normal) Black Cauldron, and it was a bit of a letdown.
And they didn't even use that for extreme!
