In DK alone we have more than 65K drugs licensed and marketed. I doubt you know 65K laws by heart vs more the general idea, groups and mechanisms, most used, specialize in some? Sorta how it works. We're not pharmacists. Hence why a pain doc might know about, say, chronic pain treatments incl the drug options? I'm not a pain doc. Here's Dr. House shrugging.

I was saying that we wish doctors and the medical field knew more about the "why" of things, not criticizing you for using the available resources. Of course I don't know laws by heart (generally), I go look shit up. Same as you. What I'm saying is that doctors here aren't going to look shit up any differently than doctors there, and that we wish there was more to it than "hey try this drug and see if it works for you." That isn't a criticism of doctors, it's a criticism of the current state of the medical field, and we wish it was more like star trek.

Dealing with chronic pain patients when there's a condition exacerbating the existing condition is difficult at best. Last one I had came with pain after a misstep after dismouting her bike, no trauma. Previously had multiple surgeries for slipped discs, ovarian cancer and also a sacral pain modulator, no bowel function and possible ileus. Screaming in pain from LLQ. Morphine ad libitum. Multiple CT and MRI. Two days later remission. No obvious reason found. Back to regular drug and management regime, and referal to pain clinic optimization. No previous drug seeking behaviour. Most chronics don't really want highs, rather they want tolerable and stable. I really wish I had that Star Trek Medical Tricorder a lot of days.

But heeeey, now I know what I want for Christmas <3

The chemo oncologist I am seeing not only looks it up on his computer doctor manual, he shows it to you while he's looking it up. Hell I am no different. I consider myself very proficient wit h the National Electrical Code , but by that I mean I know how to use the index to look up what I need to know. In fact I went to take the electrical license test in TN and part of it was closed book? I was like wtf if I don't know how many amps #8 AWG copper can draw at 75C by heart no one is going to die if I walk out to the truck to look it up.

Whelp, the hits keep coming. I apparently have signs of non-alcoholic fatty liver disease. I'm not over weight (6'2", 170 lbs, 1.88 meters, 77 KG), I don't have diabetes, nor any of the other general causes. Know what I have? Genes! This answers some questions for my family. My grandfather had a failing liver, but no one ever saw him drink. I have an uncle who had the same thing.

So now I need to figure out what the fuck to do. It's not a small thing. sigh.

Well I think the answer is obvious : Start drinking heavily! If you have to have the downside, have some fun too! I was having an issue in the past and they told me that it turned out to be , I think, an allergic reaction to all the narcotic pain meds I was on. I get genetics are a bitch , melanoma runs in my family and I have to go back to see about a spot on my left middle finger myself. Keep up the good fight mate (and get rid of that metric stuff already , geez ) ;)

TL;DR: BMI isn't a great measurement, and there are some people in the ideal BMI weight range who store a lot of fat viscerally (in their trunk; commonly referred to as an "apple" body type), which can still lead to obesity-like health conditions. The military has started using waist circumference instead of BMI to determine whether someone is in a good weight range, and others are starting to follow suit (it's a better measure of visceral fat).

If you consume a lot of fructose, try to lower your intake, as it specifically has been found to be very inflammatory for the liver. Generally any kind of processed sugars, but also fruit juice. Some fruits also skew very heavily towards fructose - those are dried dates, grapes, pears, apples, cherries, bananas, blueberries, and mangos. I would worry about cutting out anything processed before you start getting very micromanagerial about fruit, though.

If you can, you may also want to cut out alcohol entirely, though I know that's tough to do with a lot of people.

My doc said a lot of these same things. But my waisted is 31, I didn’t drink pop, I don’t snack between meals, and I eat very healthy. Maybe too much red meat, but piles of vegetables and fruit (a bonus of growing your own). It just isn’t fair. Yes, sorry, I am whining. This sucks balls.

If it makes you feel any better I would estimate like 1 in 5 scans/ultrasounds in America do NOT mention a fatty liver.

(ok actually I looked it up and a study showed like 2 in 5 scans but still a solid majority lol)

That’s one thing that surprised me in Asia, most diabetics aren’t obese or even overweight, and they seemingly eat healthy.

By seemingly I mean I suspect the rice is fucking them up and their genetics prevent them from seeing it, like white people would gain wait.

You can eat 'healthy’ and few snacks but still have constant high blood sugar, and by high,.even slightly under 100 fasting blood sugar can be enough over decades if you're also regularly and for hours, every day, at 120 130 140 after meals.

Diabetes, fatty liver in lean Asian is fairly common. Rarer among white, but that’s not really a 'rare' genetic 'problem'.

Keto , fasting will reverse that shit in a few months.

Chronic pain syndromes like neuropathy, migraines or fibromyalgia are not conditions you should expect cure or even effective treatment from medicine. They are poorly understood, the causes are often impossible to identify or treat, they rely on subjective evaluation of pain and not objective testing, doctors have no way to tell how much of it is mental. And most importantly they don't kill, don't necessarily impair or get worse, so drs can get away with just giving painkiller: tricyclic antidepressants, the gabapentin pregabalin stuff, or even opioids. Often these treatments are worse than the pain they try to relieve on the long term, and you just learn how to cope with it drug free.

Medicine just isn't nearly as advanced as the layman thinks. In the USA though if you have money your chance of getting an effective treatment is orders of magnitude higher than anywhere else in the world. Painful neuropathy (if it's autoimmune) sometime get relief from immunoglobulin infusion but you won't get that treatment anywhere other than in the USA if it's for pain only.
