Injecting directly into the wound site is probably alright for a wound like that. An intraperitoneal or intramuscular injection would be the medical term for it (depending on how deep she stuck the needle). For a wound that bad, with a limited amount of antibiotic, it might actually be ideal doing it like that. More of the medicine is kept local to the wound rather than being carried through the whole body by the heart.
