A respiratory insufficient patient where we can't get a tube in or ventilate with a mask, basically suffocating and dying if we don't get oxygen in the lungs somehow. So the cric efona, emergency front of neck access procedure is indicated. It's part of the unexpected / difficult airway algorithm. Minutes without oxygen = brain damage. And with a child, jesus f... christ, stuff of nightmares. So we practice efona, often, to be able to do this and, well, save brain and lives.
