More famously Jerry Garcia of Grateful Dead died in a California rehab facility, after stating that he believed he was just too old to detox from what he was dealing with.

one of my friends is an alcoholic, he has nearly died several times now. he has done rehab a few times, but last time was supposedly one of the best in the nation in cali. they told him, he had around 25%-ish chance of not making it though detox. that was a couple of years ago, with he health problems stacking up recently, would guess it would even greater if he tries again.

the cali rehab first was detox, then like a month+ in a shared house with 8-10 other people with lots of doctors/meetings etc. after that, few weeks in a small apartment, with only 1-2 appointments per day. my friend made it like 2 weeks at the apartment, before starting to drink again.

Watched some show a few years ago, learned that alcohol delirium tremens could be fatal. How much booze does one have to pack away to get in a state like that.

Alcohol is the drug you're mostly likely to die of the withdrawal from. I watched a documentary a while back about a guy that was a hopeless alcoholic and his mom and sister took him to an addiction specialist and while they were meeting he started to shake and the doctor told they sister "Go across the street and get him a pint of vodka now" because he hadn't had a drink all morning and he was literally going to die. He went to rehab and died a couple of weeks later even with their supervision.
Not saying it can't be negligence by the rehab, but some people are so far gone by the time they finally decide to get help that they just can't survive it. Shit is bad for you.

my friend weighs about 150lb and was drinking a 6 pack and most of a 750ml bottle of vodka every night. he would drink the remaining vodka when he woke up. when he got 1st dui, bac was .3 something and .4 something his first time he about died. later he joked, that for about a year, he was never not legally drunk.

No idea, but as someone who went dry about two years ago, you can develop what amounts to essentially an allergy to booze eventually. I started to hit this point and decided that was my stopping point. I was never really addicted, but I was overdoing it from stress peers and pain. Stopped drinking completely, outside of a half shot once in forever or beer/whine as a cooking ingredient. But at the tail end of it I definitely was becoming physically ill from the passing out and I got blackouts full on that I never got before. So for posterity, I am a ham planet and I was putting away about 8 or so bottles of wine or premix margies a week before I went into retirement. I would imagine that based on the symptoms I was having that I would have been dead inside a year at that pace. I miss the fun of being drunk, but there are gummies and I can just eat the stress and pain from now on. People act like it was some sort of super power to just up and quit without some sort of intervention or withdrawal period, but I had quit for long stretches in the past cold turkey without really any thought about it, like years at a time.
I can definitely see it killing someone who drinks when its gone full toxic for them to do so, though. Probably a combination of factors, including body chemistry/genetics in the mix, combined with age and sustained rates of consumption. No idea why I was able to drop it instantly for years all three times I did, given what I have seen others go through.

During Prohibition, they actually had an exception for medically prescribed alcohol because even back then, they knew severe withdrawal symptoms could be fatal.
