Well there's plenty of new items being added, they're just not added frequently. You cited Harkon's Blade and Spellshards, but there was what a 1.5year gap between these 2? Harkon was added very early in HoN, pretty sure during beta still, Spell Shards were added after the game went F2P or around that time.
If you want some of the more recent dota items: Smoke of Deceit(big addition for support play, early agression, ganking, lategame turn around), Tranquil Boots(cheap alternative for supports, have gone through multiple forms, similar to HoN boots in some ways), Veil of Discord(added a while ago, similar to Spell Shards in some ways, has been buffed regularily), Rod of Atos(fairly niche item but pretty strong nowadays, just tends to be ignored in favor of sheepstick for heroes that can farm money and for more utility oriented stuff for those that can't), Ethereal Blade(lategame nuking item, synergy with int nukers as well as other utility effects), Drums of Endurance(utility/support, great buildup), Ring of Aquila(a bit meh but easy buildup). All these were added in the past 2-3years as far as I remember.
There's also constant tweaking of existing items so they become more useful or more interesting, and everytime a hero gets a new aghanim upgrade it's a pretty interesting change.
I think you might be bored with the game which creates some bias but overall, the game is updated and changes a lot. It's not weekly patches but it shouldn't be, not even every 2weeks, changes need to settle and the meta needs to evolve before you can make changes that are actually useful. Even then every 3-4months there's fairly big patches that change quite a lot of stuff, and new heroes are added every other major patches(so like every 6-8months, might be longer sometimes). Sometimes new items are also added.
