November last year, I admitted myself to my local ER, with an infected big toe on my right foot. I had smashed it a couple months prior, helping my brother move a heavy dresser, and initially thought I was just gonna lose that toenail. I've had nerve damage in my toes ever since I was 18, I weighed close to 400lbs at the time, and worked 12 hour shifts, overnights at a convenience store, wasn't allowed to sit, and wore the worst shoes that have ever existed. After about six months of that job, my toes went permanently numb, worse in the big toe, getting better as you move into my smaller toes, so I didn't notice that what I thought was just a black toe nail, was actually a completely broken distal phalanx. When the toe nail finally fell off, I thought I had an ingrown toe nail that had formed underneath, kept it bandaged for awhile, until I noticed one day I had some red streaking and swelling in the foot. At the ER they confirmed a bone infection and admitted me for further tests and a likely amputation. A1C tests came back showing my blood glucose levels had been in the 240 range for at least the last 6 months. I spent a week in the hospital, had my right great toe removed, a picc line put in, and got sent home for six weeks of IV antibiotics.

I took the diabetes diagnosis to heart, immediately changed my diet, started exercising (as much as I could), and dropped from 270lbs, currently down to 225lbs. I'm on 500mg of Metformin twice daily, and haven't had to use insulin since I've been out of the hospital, and have successfully kept my blood sugar between 90 to 130 without much issue. Beginning of January, a follow up with my podiatrist showed that my second toe was starting to develop fractures, within a week I had a blister on it, which ultimately led to having it removed to the first joint. That partial toe, never looked right after, was red the entire time, and after they took stitches out, the right side of that toe just wouldn't heal, so I just had the rest of it removed towards the end of February.

So this whole time, maybe starting a month or two before me, one of my best friends dads has been undergoing the same process. They removed his big toe, second toe got messed up, and they did a partial amputation. His got worse, he bandaged his partial poorly, leading to his 3rd toe getting infected, and they took all the toes on his right foot off around the time they did my partial. Now, he's rubbed a blister on his foot, and they are working towards preventing infection of his metatarsals.

My podiatrist says my last 3 toes are good pink healthy toes, and that I shouldn't have to worry as much about running in to possible issues with them, but given whats happened to my friends dad, I'm paranoid. I've lost 2 toes and am on leave from work till at least April 23rd. I've talked extensively with my podiatrist, endocrinologist, and primary care providers, who all assure me that I'm doing great, and have no recommendations on what to do better or differently, but still feel like I'm just gonna wind up on a surgery table again until I don't have a foot.

I think my foot looks worse because of the rightside heart failure which causes it to swell too. That picture scares the shit out of me though given how I know mine looks.

Slight temp, girlfriend had it first. Had been off the Norco for six days when symptoms hit, so it's not that.

No blood test to confirm this time but it feels identical to the flu I had last month, which was lab confirmed.
When I used to use up my norco up to a week early my symptoms start after 12 hours and peaks in 48. By 5-6 days if I ran that long without it they start going away.
Starts with feeling very tired and runny nose, By day 2 I start sneezing. I am VERY curious about the mention of viral relations to this as I've noticed Shelly AND the cats seem to get bad sinus shit as well when I start doing it. Maybe it is just coincidence? Thankfully the pain clinic has upped me to where I don't run out as much and stay in therapy levels more now. They had raised my Gabapentin to 1800mg daily too but I stopped seeing benefit when I was doing the 300mgx3 daily so now I am back down to 1200mg daily or 300mg at the same time I take my norco.

What's bullshit about it? I was sweating and I could feel my heart jumping and racing then normal and repeat for hours, it usually lasts minutes tops. They have only caught it doing it a few times, first time in 2015 when I had the embolisms( it has been going on since probably 2013 but I never went to a doctor about it ). I went about midnight and it was gone in a hour, in the morning the cardiologist literally said if I had called when it started which his office was still open, he could shock me straight and I wouldn't have to sleep in the hospital all night.

Me saying bullshit was that we don't HAVE to have cardiologists in the ER to cardiovert. If it could turn south fast, then yeah, we would like to have them at bedside. But, we as RNs and all the physicians/providers in the ED are capable of cardioverting and pacing.

What was your rhythm? It kind of sounds like paroxsymal AFib or SVT. But, without a picture of your EKG, hard to tell.

BTW, how are your sodium levels with that much Gabapentin? That is a massively high dose. You must got some serious comorbidities.

You should try to switch to Lyrica instead of Neurontin. I did last year and it's world's better. More effective with fewer side effects.

Main side effect that bugged me with gabapentin was memory impairment. Lyrica still does it a bit but not nearly as much.

I'm kind of scared to switch. I had this sensation for over a year where it felt like I was walking through burning kerosene and once I got above 600mg/day it started going away. At 900 it is reduced to s slight tingle. I was so happy to have that 80% gone I am hesitant to switch to anything. I hate new medicines because I take near a dozen different ones a day and most of them multiple times. I get anxiety at this point when they add/change a medicine because if one domino falls, I feel like the rest will too. The gabapentin is working, maybe not 100% but life is much better than it was say 6 months ago where I felt like I would spontaneously combust at any given second.

If it's good enough then yeah, maybe don't switch. I have literally never seen someone regret the switch tho, even having talked with probably two or three dozen other patients about it between online and my extended hospital stays.

Only thing is there is no generic version yet, so some insurances are pretty hesitant to allow the switch.

Is it dark/black stools or is it bright red? If it's a lot and it's been happening for the last month, then yeah, I would be quite unhappy as well. Reminds me of a patient we JUST had who had chest pain in the gym and decided to go home and Google search the symptoms. Finally decided to come to the ER.

Yeah...he dropped dead in our waiting room. Complete cardiac arrest.

It sometimes is pinky and other times darker and mucusy. Either way, I got the tests results back and my GP upgraded my referral to "urgent" with a specialist I will hopefully see this week. They also found white blood cells in the stool which I guess indicates potentially some kind of infection. Apparently the lower back pain I've been having may also be related. More to come unless I die!

Man I hate hearing that, where are the tumors at?

my oncologists were hesitant to name exact lymph nodes before the radiologist finished his report, but they said one is definitely in my axillary lymph node, another is likely my paraaortic. but despite being very skilled oncologists, they're not trained enough on radiology to be certain, i guess. i think their guesses would probably have been right, but they didn't want to take any chances of misleading me.

based on my pain i think another one is back in my left retroperitoneal area like before

Fuck Cancer.

Sorry to hear about that, pharmakos. Never easy. Had a patient last week who came in with right quadrant pain. Hours of tests later, we find out it's a neoplastic malignancy that has metastasized.

As with everything, I do recommend you being an advocate and getting a second opinion. It never hurts.

Cancer has taken too many in my family, and I watched them all slowly disintegrate. Sometimes I felt like the coward because while they were fighting for their lives, on some days I just didn't have the emotional fortitude to look at them.. I literally have carcinophobia from seeing so many family members die from cancer that anytime I have an odd pain (which happens frequently at 45) I'm at my doctor thinking "cancer".

My only suggestion, and it's what I've told myself I would do, is that I won't die shriveled in a hospital bed. I'll be going out in a blaze of drug abuse somewhere in Asia.

Really sorry to hear this man... How old are you btw?
