Yes you take it to the cave in chapter 2 and reveal it as if it were a an item with a jawbone/rib applied. basically its a crafting tool that lets you play around and guarantee an abyss mod

If there's an empty affix slot, it exalts it, if the item is full, it removes one to make room. It kinda screwed me because it removed a +76 phys mod on my bow to make room for a +24 phys/40 accuracy :(
