There are a few really good doctors though... that's why they are usually too busy to see you.


It's pretty much the difference between a cook and a chef.. a cook just follows the recipe.. a chef can think creatively on the fly.

I tried acupuncture once for my back, and it didn't do shit. But then again, I've been to plenty of doctors who's treatment didn't do shit either, so I'm not going to pass judgement. I just think, if it is legitimate, why can't they provide a better explanation than 'the needle un-clogs your mana'?
Back pain sucks. For generic run of the mill chronic back pain, I can give you an exercise video and tell you to do what's on the video and it's just as good as any other treatment (be it the cadillac physical therapy plan, medications, etc.)

I don't think doctors are really any dumber than other people, but the problem is that they're not any smarter either. We're imbued as kids with the idea that doctors are all incredibly smart and wise, that they're all experts who know exactly what they're doing, etc. So we get pissed off that like everyone else, a lot of them are stupid, or assholes, or ignorant, or just plain shittty at their job. It can also be hard to separate the good from the bad, since no doctor is going to admit that maybe they're wrong, or that they just don't know.
I don't think you know a lot of doctors. Sure, many of them are assholes. But I'm always upfront and honest with my patients. Just tonight in my ER I had a girl complaining of irregular periods, pain around the left side of her (7 year old) c-section incision, and then some random right flank pain. No elevated white count, her urine was normal. I told her I really don't know what's causing her new left sided pain, and that it's possibly some problem with her ovaries, adhesions, or any of a number of other things. I explained I'm not an ob-gyn, and that honestly most of their knowledge base is completely foreign to me and that she needs to see one of them. I told her that what she has is not an emergency so there's nothing I need to do beyond tell her to see an ob-gyn tomorrow. I told her since she's already in the ER, I can do a CT scan of her abdomen and pelvis to investigate the possibility of an ovarian mass, but I explained the risks and benefits of a CT scan.

I'm like that with all my patients. Just honest. A lot of practitioners do stupid shit like give everybody antibiotics despite that they don't need them. I just explain why they don't need them and I rarely have people get upset with me. And occasionally I do prescribe them when they aren't probably needed, and even in those instances I tell them they likely don't need them, but explain why I'm giving them to them anyway. For instance, tonight I had an elderly lady with COPD and diabetes with a 2 week history of a cough. Chest x-ray was normal. Likely bronchitis, 90% of which are viral. But she's high risk, and a bacterial infection could cause a COPD exacerbation and put her in the hospital and possibly kill her. So I went ahead and gave her antibiotics, but explained they probably won't help, but because she's so high risk I thought it was prudent to use them.

So yeah, some docs may be shitty, but not all of us are. I think it truly depends on how interested they are in the academic side of things, because I love understanding exactly why I'm doing something, instead of just following guidelines. And then I can use that as a small teaching point to my patients, and then that helps them trust you that you are doing what's right for them. ... Unless the patient is a fucking retard, in which I let them know that you do not come to the emergency room of my hospital and complain of chronic back pain...

BTW, your comments about doctors not being smarter than most people. Sorry. You're absolutely wrong. Remember the mean IQ is 100 or so. That's a pretty fucking stupid person. And half the population is below that! You aren't finding any doctors in the USA with an IQ of 100. The vast majority are a whole lot smarter than the vast majority of the population. I'm not saying every doctor is smarter than any other academic (PhDs, etc), they're probably all around on par with each other, but to sit there and say that they're average is completely fucking retarded. You have no idea the amount of work it takes to assimilate retarded amounts of knowledge and be able to sift through it and apply it usefully in the window of a single second when it actually matters.

Besides, you're a fucking socialist retard who is inherently going to dismiss anyone who has actually worked hard and achieved.

Regarding how acupuncture works, the concept is many ailments are a result of blocked energy flow (chi). The needles are inserted at specific points to remove the blockage and things start flowing right again. Sounds utterly fucking ridiculous but for some things it does work. Why? I guess there's just some things science hasn't yet explained. I went into it not expecting anything and was surprised with the results. I also felt rather well at the end of the treatments. That would last a few days, sometimes less if I got my jimmies all rustled.

My friend wasn't allergic to shellfish, just fish. Needed Benadryl kind of allergic. Doctors couldn't figure out what was wrong with him. He had all kinds of stomach issues and pains. Now he's fine. He's not even allergic to his dog anymore.

I'm not against science and medicine. I still think doctors are fucking dumb.

There is absolutely zero evidence supporting the existence of your supposed energy force (chi). So tell me how can acupuncture supposedly restore the flow of a force that has never been proven to exist?

It has not been around for thousands of years as you may have read from some retard trying to get you to pay so that he can stick needles randomly in your back (there are no such thing as acupuncture points, no such anatomical entity has *every* been described.) In fact, early Chinese books of medicine describe the use of needles to lance boils (abscesses). The idea of acupuncture was used by Chinese street trash as a form of cheap, but useless, medicine several hundreds years ago. And China even banned the practice of acupuncture because it was so stupid and useless. It wasn't until Mao needed a way to provide cheap (but useless) healthcare to his large population that acupuncture started to be used again. And much of what you think of as "ancient acupuncture" was literally made up on the spot in recent years.

It's bullshit.

When you read the studies, you'll note that you'll only ever find retarded studies that show *subjective* improvement compared to no treatment or obvious placebo (ie, a shock to the wrist compared to acupuncture.) You'll find studies that show it doesn't matter if you stick needles in acupuncture points or not, the effect is the same. And you'll find studies that shows that just poking someones back with toothpicks at random locations yields the same supposed benefits as acupuncture. And you'll find studies that show if practitioners are courteous to their patients they'll get better results with acupuncture than practitioners that are cold to their patients. And you'll even find studies that show that if you're older and have a beard, you get better results with acupuncture than if you're a young female practitioner.

What you won't find is any large, powerful double-blinded randomized placebo controlled study that demonstrates an *objective* improvement after receiving acupuncture treatments. There was a great study of asthmatics receiving acupuncture. They reported *subjective* improvements after receiving acupuncture compared to placebo, but they didn't move any more air (objective). By definition, placebo.

In fact, all of this bullshit alternative medicine that prays on new age retards only ever treats subjective complaints, most commonly pain. Gee. I wonder why.

You have no idea what you're talking about. If most doctors "followed the book" we'd probably be better off. The problem is a lot of doctors aren't aware of what the book says. The worse problem is all the retarded nurse practitioners that are given free reign to do whatever they want and some money hungry MD just blindly signs behind them. Some of the shit they do is so fucking stupid that I'm amazing they can get dressed. Their complete lack of training is so pathetic.

I don't think it's NPs, I think it's people in general. Some doctors are really fucking dumb, just like some nurses are really dumb. In my clinical last week, one nurse actually said, "I didn't know what IT band was, so I googled it and learned something new." I can't believe she passed an anatomy class, lol. But, I know a couple of doctors who don't do research on current medicine which makes them dangerous depending on what drugs they prescribe (eg Retrovir).

I'll have about 596 clinical hours when I finish my master's in nursing this Summer. I've been a nurse for 4.5 years but that experience doesn't prepare you at all to be a provider. I specialize in Psychiatry which is a totally different than being an FNP. I don't know how an FNP can be ready to practice after the same amount of hours I complete considering I am limited to treating DSM-IV disorders where someone working in family practice basically has to know how to treat everything. I definitely see how this is a major rustle point. Ultimately, it is up to the NP to not bite off more than they can chew and consult with their collaborating physician (a big part of being an NP). I know some really, really bad nurses and I know some really, really bad doctors. Some of it is stupidity/ignorance and some of it is being lazy and just not giving a shit. I guess it rustles my jimmies when someone says anything bad about nurses and I feel compelled to defend them, however, I definitely see where you're coming from (because I know some many fucktard nurses and a couple of my classmates can barely operate a computer).

I just wonder if you have to be in the medical profession for a long while to find the ability to cut all the shit and be honest with patients. I generally trust doctors, but I would sure as hell trust a doctor who told me flat out that he didn't know (or that I had no business in the ER.)

Well yeah that's my point. There are good doctors, terrible doctors, kind doctors, asshole doctors, everything in between. I don't know maybe I'm completely off base, but it seems that a lot of people, especially if they're younger or haven't had significant interaction with medical professionals beyond the occasional flu shot or whatever, have a somewhat idealized view of the profession, take everything their doctor says as gospel because 'the doctor said it, and they would know!', and don't seem to realize that doctors can suck at their job just as much as anyone else. I'm not saying they all do, just that they vary in competence as much as any profession, so you have to be ready to get second opinions and maybe look into things a little yourself if their treatment doesn't work or what they're saying doesn't add up.

Although I see the same thing a lot when people talk about lawyers and professors, but doctors in particular seem to be put on a pedestal by a lot of people.
